If you would like your ASFLC involvement noted on your official FLC transcript, then you must fill out this form and submit directly to the Advisor no later than 30 days prior to the end of the semester.
All requirements must be met in order to have "Recognized Student Government Officer" noted on your transcript. If you have questions or need additional information, please contact the Advisor directly. Thank you.
