Letters of Accommodation Request Form

Before You Fill Out This Form

This form is for requesting Letters of Accommodation (LOAs) for classes at Folsom Lake College. To request LOAs, you must be an active DSPS student who has:

1. Applied to DSPS at FLC
2. Completed DSPS Intake with a DSPS Intake Counselor

If you have not applied to DSPS, completed intake, or checked in with DSPS within two consecutive semesters, you must apply or reapply to DSPS before requesting LOAs.
