Before You Fill Out This Form

This form is for requesting Letters of Accommodation (LOAs) for classes at Folsom Lake College. To request LOAs, you must be an active DSPS student who has:

Applied to DSPS at FLC
Completed DSPS Intake with a DSPS Intake Counselor

If you have not applied to DSPS, completed intake, or checked in with DSPS within two consecutive semesters, you must apply or reapply to DSPS before requesting LOAs.

*Please note: LOA’s are fulfilled in the order they are received and will be issued by the end of the 2^nd week of instruction. Students utilizing DSPS testing accommodations are required to review and sign the DSPS Test Proctoring Services Agreement as part of the Letter of Accommodation (LOA) request process. The agreement outlines DSPS testing policies, procedures, scheduling expectations, and academic integrity guidelines for students using DSPS Test Proctoring Services.
