E-FORMS

Many student petitions and forms are available in eServices as an eForm where the approval process is done electronically. When the eForm is completed by the student or college counselor, it will route to the appropriate instructor/staff/office for processing. Within eServices, click on the Other Resources tile and select eForms drop-down choice to access eForms:

  • Course Repeat Petition – use this form when petitioning to repeat a class (after already passing the class or after three previous attempts in Los Rios). View more information
  • Exceed Unit Petition – use this form to exceed the district unit load limit (18 units in fall/spring and 8 units in summer). Complete the eForm after you are enrolled in the maximum unit load through the college were you are adding the class that exceeds the unit load. You must then meet with a college counselor for approval. For adding FLC classes, call Counseling at (916) 608-6510.
  • Late Add Petition – use this form to add a course after the deadline.
  • Late Drop Petition – use this form to request a drop for refund, no notation, excused withdrawal or withdrawal notation after the deadline.
  • Loss of Priority Registration/CA Promise Grant Appeal Form - students with more than 90 degree-applicable units completed within the Los Rios Community College District and students on probation or dismissal status for two consecutive major terms on probation will no longer be eligible for priority registration. In addition, students with two consecutive terms (fall and spring) of cumulative GPA below 2.0, and/or cumulative course completion of less than 50% may result in loss of Promise Grant fee waiver eligibility. Students may appeal the loss of priority registration or fee waiver eligibility each term for the reasons listed on the appeal form only. You must provide the required documentation for consideration of your appeal.
  • Major and Ed Goal Change Form – use this form when updating your FLC major or educational goal.
  • Time Conflict Petition – use this form to request enrollment in a course which occurs at the same time as another course. Make sure to petition through the college that will be enrolling you in the class with the time conflict.

AB 343 Non-Resident Tuition Exemption form – Students who have a special immigrant visa and settled in California upon entering the U.S. shall be exempt from paying the non-resident tuition fee for the length of time he or she lives in this state up to the minimum time necessary to become a California resident.

AB 540, AB 2000 & SB 68 Non - Resident Tuition Exemption form – Individuals who have met certain criteria based on previous education within the state of California may qualify for non-resident tuition exemptions provided under State law.

Add Drop Request – use this form when you are unable to add or drop classes online via eServices. (You must submit using your Los Rios Gmail account).

Change of Data form – use this form when updating name, address, etc.
For Name Changes: Please provide at least two forms of government issued I.D. (one with new name and one with old name) with your request OR a copy of the official name change document from the court.
For Social Security Number Changes: Please provide copy of SSN card. (You must submit using your Los Rios Gmail account).

Course Repeat Petition – use this form when petitioning to repeat a class, either after standard grade or 3 previous attempts of substandard grades.

CSU GE/IGETC Certification Request – use this form when needing a CSU GE or IGETC certification for your transfer institution. This must be done with a counselor. The counseling department phone number is (916) 608-6510.

English Prerequisite Clearance form – enrollment into ENGWR, ENGED and ENGCW classes is blocked unless the student has met the prerequisite through Los Rios placement based on the admissions application or completion of Los Rios course that is the prerequisite. If prerequisite course is taken outside of Los Rios and is on the approved external equivalency list - or - AP test scores provide proper placement, this form should be used. Email form with official or unofficial transcript or AP test score to Assessment@flc.losrios.edu. If the class/college is not on the external equivalency list, student must go through Prerequisite Challenge process through instructional departments (see form below). Enrollment Verification Request – to be used when requesting enrollment verifications. (You must submit using your Los Rios Gmail account).

Math Prerequisite Clearance form – enrollment into Math classes is blocked unless the student has met the prerequisite through Los Rios placement based on the admissions application or completion of Los Rios course that is the prerequisite. If prerequisite course is taken outside of Los Rios and is on the approved external equivalency list - or - AP test scores provide proper placement, this form should be used. Email form with official or unofficial transcript or AP test score to Assessment@flc.losrios.edu. If the class/college is not on the external equivalency list, student must go through Prerequisite Challenge process through instructional departments (see form below). Pass/No Pass Petition – use this form to change grading basis of a class from letter grade to Pass/No Pass must be turned in to A&R by published deadline each semester.

Prerequisite Challenge Application – use this form when challenging the prerequisite of a course. View prerequisite verification.

Residency Reclassification Application (and Instructions) – for students who are requesting a residency reclassification.

Request to Withhold Directory Information – use this form to "opt-out" or request to withhold disclosure of student directory information, per FERPA (Family Rights and Privacy Act) provisions.

Student Consent for Release of Records form – use this form to approve release of your educational records to a third party, such as parents. This form allows for a one-time release of specific student records to a third party.

Student Petition – use this form to request an exception to college policies such as drop after deadline, refund exception, etc.

Substandard Grade – Repeat Notification Form – use this form to request that a course repeat be noted on your transcript (when course is repeated at a college other than FLC).
